Our administrative staff is ready to help you with questions about scheduling, financial policy and insurance, to make that part of the process as simple as possible.
Patient Forms
We require that you pre-register with our office by filling out our no print, secure online Patient Registration Forms. After you have completed the forms, please make sure to click the Complete/Send button at the bottom to automatically send us your information. The security and privacy of your personal data is one of our primary concerns and we have taken every precaution to protect it because the foundation of a long term relationship is established at before your first visit to our practice.
New Patients
New patients are required to complete all registration forms prior to arrival. We strive to be a paperless practice, want to answer any coverage concerns accurately, as well as reduce unnecessary contact for everyone and pre-registration is critical to achieving those goals. If you require assistance in completing forms electronically, please let our Business Office know at the time of scheduling.
Established Patients
Once established, you will be notified with a "Check In" text prior to your appointment when annual updates are required. Established patients who have created portal accounts can access the portal (link above) to view appointment history, see forms that have been completed & those that are coming due for update, and more at any time. If you do not yet have portal access or need additional family member access (patients 18 & over require their own portal), please contact us during regular business hours. Payments to your family account can be made by using any of our eStatement options or request payment portal set up.

Being well-prepared for your appointment will ensure that the doctor has all of the needed information to provide the best possible care for you. It also will help relieve any unnecessary anxiety you may be feeling. Also, take some time to review our staff page and familiarize yourself with the doctors. Our goal is for your first visit to be one of comfort and acknowledgement of your personal dental needs.
We will take the required time to properly diagnose your immediate dental concerns, review your past medical and dental history, and create a treatment plan that provides for your optimal dental health. With your cooperation, we will complete a thorough comprehensive dental evaluation.
